- Hourly Fee : $160 per hour

- 1st Meeting: 1 - 2 hours
- Additional Meetings: 1 hour

- Private Payment / Co-Payment is due at the time of service.
- All sessions end 10 minutes early for therapist to chart discussion.

Insurance & Private Payment
If you are going to use insurance for payment, it is important that you check your benefits. Often, mental health benefits are different from major medical benefits. While we may check your insurance, this is not a guarantee of payment.
- We are preferred providers for BCBS, ODS, MHN & TriCare.
For all other plans we are "out of network". This may make a difference in insurance coverage.
- Your insurance provider's phone number is on your insurance card. Call and ask:
- Do I have mental health benefits?
- How many sessions are allowed per year? How many have I already used?
- Does my plan allow 1 hour individual sessions (CPT code 90806 )?
- Does my plan allow 1 hour parent meetings (CPT code 90846)?
- Does my plan allow psychological assessment? (CPT code 96101)?
- What is my copayment/coinsurance?
- What is my deductible?
Phone Call, E-Mail & Fax Billing
Phone calls, faxes and e-mails that require more than 10 minutes for reading & responding will be billed at our regular rate. This is not reimbursed through insurance. Fees for Missed Sessions and Arriving Late
- If you are unable to attend, please cancel your session 48 hours in advance.
- If you cancel with less than 48 hours notice or do not keep your appointment 2 times, you will be referred to another provider.
- If you are late to an appointment or have to leave early, you will be billed for the full time allotted for your session. The session will end at the scheduled time.
Free Services
New Leaves Clinic believes in giving back to the community. Staff provides continuing education & training programs for community agencies. However, New Leaves Clinic does not provide free services for direct clinical work.
